The patient portal is a secure online website that gives patients convenient 24-hour access to personal health information from anywhere with an Internet connection.

Using a secure username and password, patients can view health information such as their Visit Summary, Medications, Appointments and Billing Statements. Patients will also be able to request, reschedule or cancel an appointment from the patient portal as well as request PHR/PHI. Use the link below to access your patient portal.

What is the Patient Portal?

A patient portal is a secure method for patients to access, communicate and receive information related to their electronic health record (EHR) online. Patients who are web enabled may access the portal securely with their username and password. The Patient Portal is an easy way to get in touch with our care team. It is a secure, internet application that allows you to:

  • Access to Clinical Summaries from each visit at Esperanza Behavioral Health and Services (EBHS).
  • Access to personal health information contained within the EHR which includes information about medications, medication authorization processing, and treating provider.
  • Request information such as an appointment with a EBHS provider, or reschedule or cancel an existing appointment.


How can I get a Patient Portal account?

All you need is a current email address. At your next appointment or over the phone, ask a front desk or nurse to enroll you in the Patient Portal.


Why should I use the Patient Portal?

Sometimes, it isn’t convenient to call our care team. Other times, you may have a non-urgent question and think an appointment isn’t needed. With the Patient Portal, you can review visit summaries, results and appointment information on-line at your convenience.


How do I get to the Patient Portal?

A EBHS staff member will enroll you and activate you as web enabled in our system. They will provide you with instructions for how to log on to the patient portal via the Internet. It is as simple as clicking the “login” button and typing the username and password that is provided.


How do I know my health information is safe?

EBHS has made significant investments to secure your information. We currently meet all regulatory requirements for security of our network and data. Information on the Patient Portal is protected as part of your electronic health record.


Can I use the Patient Portal if I have an urgent need?

The portal is to be used for obtaining general information about your healthcare. It is not to be used for urgent questions or emergency situations that require contacting your healthcare team.


Can I sign my whole family up to be enrolled in the Esperanza Behavioral Health and Services Patient Portal program?

Yes, once our family member is a patient of EBHS they will be given an individual patient portal account.


What happens if a child has multiple guardians that need access to a child’s account?

There will only be one unique username and password assigned for each child. It is up to the child’s guardians to determine if they want to make the decision to confidentially share the account information to gain access to the child’s health records.


Can I print from the EBHS Patient Portal?

Patients are able to print information from the portal. Anything you print from the patient portal is your responsibility. To protect your privacy, keep these papers safe. Do not leave printed materials out in a public area for others to view.


What do I do if I forget my username or password?

It is your responsibility to keep your username and password a secret. EBHS is not responsible if you inadvertently misplace or lose your username or password and someone else uses the password and views your private information. EBHS will never ask for your password. If someone calls and asks you for it, do not give them your password. In the event you forget your password, select “forgot username” that is on the login section of the home page. You will be prompted to enter your e-mail address. If you select “forgot password” you will be prompted to enter your first name, last name, date of birth and zip code that you entered, when your account was created. For security purposes, you will be emailed a verification link and asked to confirm your new password again in order to complete the password reset.


What do I do if I have trouble logging in?

In the event that you forget your password or have trouble logging in, you can go to the EBHS main website at and find the Patient Portal link at the top of the home page. Click on the link and provide your contact information and request technical assistance.
